Applicants are also encouraged to submit proof of income and/or past due bills to support their application

If selected, our monthly beneficiary will choose between a gas station or grocery store gift card valued at $250

Grocery store gift card options are: Kroger, Meijer, Walmart, or Aldi

Gas station may be selected by the beneficiary based on their location
